This book offers an original and detailed reading of Plato's _Republic_, one of the most influential philosophical works in the emergence of Western philosophy. Bronchiolitis obliterans is the most common pathologic finding on biopsy, characterized by fibrous granulation tissue, which obliterates the lumen of the bronchiole. Clinically, in the absence of tissue for pathology, BO syndrome refers to a progressive irreversible drop in the forced (...) expiratory volume in 1 second. Recently, a broader definition of chronic rejection, termed "chronic lung allograft dysfunction", has been used to encompass a more inclusive definition of posttransplant dysfunction. Recently, the lung transplant community has come to realize that chronic rejection may be the final common result after repetitive epithelial insults. Acute rejection, infection, and alloreactivity to mismatched HLA antigens are a few of these insults that damage the surface of the bronchioles. Recent evidence of autoimmunity to the normally hidden structural proteins collagen V and K-α1 tubulin have been correlated with a BO phenotype as well, perhaps correlating the epithelial damage with a mechanism for developing BO lesions. Many immunomodulatory medications and treatments have been studied for effectiveness for the treatment of chronic lung allograft dysfunction. New drugs, which more precisely target the immune system, are being developed and tested. Further study is required, but recent advances have improved our understanding of the pathogenesis and potential intervention for this common and deadly complication of lung transplantation.Keywords: lung transplant, bronchiolitis obliterans syndrome, rejection. A new criterion is introduced for judging the suitability of various fuzzy logics for practical uncertain reasoning in a probabilistic world and the relationship of this criterion to several established criteria, and its consequences for truth functional belief, are investigated.

According to both deontologists and consequentialists, if there is a reason to promote the general happiness – or to promote any other state of affairs unrelated to one's own projects or self-interest – then the reason must apply to everyone. This view seems almost self-evident; to challenge it is to challenge the way we think of moral reasons. I contend, however, that the view depends on the unwarranted assumption that the only way to restrict the application scope of a reason (...) for action is by restricting it to those agents whose interests or projects are involved in the reason. In fact normative theories may coherently restrict application scopes in other ways. Thus we must take seriously the possibility that the reason to promote the general happiness, although genuine, does not apply to everyone. (shrink)

Chalmers' (2018) meta-problem of consciousness emphasizes unexpected common ground between otherwise incompatible positions. We argue that the materialist should welcome discussion of the meta-problem. We suggest that the core of the metaproblem is the seeming arbitrariness of subjective experience. This has an unexpected resolution when one moves to an interventionist account of scientific explanation: the same interventions that resolve the hard problem should also resolve the meta-problem.

Over the past few decades, there has been increasing attention focused on the ethics of health research, particularly in low- and middle-income countries. Despite the increasing focus on the literature addressing human protection, community engagement, appropriate consent procedures and ways to mitigate concerns around exploitation, there has been little discussion about how the duration of the research engagement may affect the ethical design and implementation of studies. In other words, what are the unique ethical challenges when researchers engage with host (...) communities for longer periods (10 years or more), and what special considerations does this time commitment generate when applying ethical principles to these kinds of studies? This article begins to outline key areas of ethical concern that arise during long-term, sustained research activities with communities in low-resource settings. Through a review of the literature and consultations with experts in health systems, we identified the following key themes: fair benefits and long-term beneficence; community autonomy, consultation and consent; impacts on local health systems; economic impacts of research participation; ethical review processes; and institutional processes and oversight within research organizations. We hope that this preliminary exploration will stimulate further dialogue and help inform ethical guidance around long-term research engagements in the developing world. (shrink)

What, ultimately, is there good reason to do? This book proposes a unified theory of agent-dependent reasons and agent-independent reasons. It holds that principles which assign reasons to agents are valid if and only if they make maximally good sense in the light of relevant data and background theories. The theory avoids problems encountered by views associated with Nagel, Parfit, Brandt, Hubin, Gert, Baier, and Tiberius, amongst others. By what criteria should a normative theory of ultimate reasons be judged? Plausible (...) meta-level criteria emerge from a process of identifying the criteria that have been used, sometimes unwittingly, by various theorists; categorizing and evaluating the criteria in the light of each other; and proposing revisions on that basis. This method escapes the drawbacks of rival approaches, such as those associated with Parfit, Gert, and Darwall. Background: Older, frail patients with multimorbidity are at an especially high risk for disease severity and death from COVID-19. The social restrictions proved challenging for the residents, their relatives, and the care staff. While these restrictions clearly impacted daily life in Norwegian nursing homes, knowledge about how the pandemic influenced nursing practice is sparse. Aim: The aim of the study was to illuminate ethical difficult situations experienced by Norwegian nurses working in nursing homes during the COVID-19 pandemic. Research design and (...) participants: The research design involved semistructured individual interviews conducted with 15 nurses working in 8 nursing homes in 3 health regions in Norway, within both urban and rural areas. Ethical considerations: Oral and written information about the study was provided before the participants gave their written consent. The transcribed interviews were de-identified. The study was approved by the Norwegian Centre for Research Data. Findings: Four ethical difficult situations were identified: (a) turning the nursing home into a prison; (b) using medication to maintain peace and order; (c) being left alone with the responsibility; and (d) s. impact on decision-making. Conclusions: The nurses’ ethical challenges were intertwined with external factors, such as national and local guidelines, and the nurses’ own internalized factors, which were connected to their subjective professionality. This duality inflicted emotional distress and gave nurses few opportunities to perform nursing in a professionally sound and safe manner. (shrink)

Rule utilitarianism as commonly formulated has the undesired implication that in case wrong has been done, Any act whatsoever is right. This is due to the fact that in setting the criterion for a set of rules which confers rightness, Rule utilitarians fail to require that obedience to the rules in situations of less than universal obedience must result in the most good attainable in those situations. An attempt is made here to examine the analysis three political thinkers - Thomas Paine, William Godwin, and Jeremy Bentham have offered of the idea of equality. The inquiry underrtaken is philosophical and not historical in character, since no attempt is made either at tracing the influence ht the biographical-cum-intellectual level of one of them upon the other or at treating their ideas on equality as born out of their preoccupation with the same problem to which they give various answers (...) and which can therefore be considered within a single overall framework. Instead, each thinker is considered independently. And a study is undertaken of the way he understands equality, the way he justifies it, the sort of equality he considers most important and his reasons for this, the area of life he takes the idea of equality to illuminate, the manner in which he relates it to other ideas, etc.. In each case, a fairly coherent philosophy of equality is sought to be constructed out of their respective writings. These three thinkers are selected for two reasons. The idea of equality looms quite large in their thought; as such, it was felt that looking at their writings from the standpoint of equality may illuminate their certain features that may otherwise remain obscure or relatively underemphasized. Further, as they consider equality from different philosophical positions it was believed that a critical examination of their writings could, perhaps, point to the general merits and limitations of their respective positions in terms of equality. in chapter I, it is argued that there are distinct views of God discernible in Paine's writings, and that corresponding to them are three distinct views of equality, though only two of them are discussed by Paine at any length. in chapter II, it is argued that, though Godwin begins as a thoroughgoing rationalist, three are shifts in his general philosophical position, and that, with each shift, his ideas on truth, rationality and equality undergo important changes. Finally, in chapter III, it is argued that there are two distinct theories of equality in Bentham and that tension between them remains unresolved. (shrink)

W. D. Ross’s ethical theory requires us somehow to compare the metaphorical “weights” of different prima facie duties, but it leaves mysterious how this might be done. The formulation of a procedure to achieve such a comparison would be desirable on practical, theoretical, and pedagogical grounds. I formulate a procedure that is congenial to Ross’s theory. Central to my procedure are instructions to characterize the weight of each prima facie duty with respect to (a) the general stringency of this kind (...) of duty, (b) the stringency of this particular duty relative to other duties of its own kind, and (c) the degree to which the duty specifically demands the particular action that it favors in a given case. The procedure leads to a determination of one’s actual, all-things-considered duty in some cases but not in all. (shrink)

It is often taken for granted that there is a crucial dichotomy between positive science, with its interest in what is the case, and morality, with its supposed interest in what ought to be the case. This assumption takes its departure from a belief in the notion of unconditional or categorical obligation or ?the moral? as ?that whose nature it is to be required or demanded?. The notion of unconditional or categorical obligation, together with the assumption that there is a (...) dichotomy between considerations of what is the case and what ought to be the case, however widespread and entrenched this notion and assumption might be, are logically confused. But to demonstrate this logical confusion is not the only task confronting ethical theory. Another, and often neglected task, is to draw attention to the ethical origins of this confusion in a particular way of life. Hegel's distinction between Moralität (Moral Life) and Sittlichkeit (Ethical Life) is of critical importance here. The obstacle to the development of ethical theory, then, is not some abstract ?logical confusion?, but is, rather, the way of life (Moral Life) of which this confusion is a natural expression. The book argues for realism defined as the assertion of the self-subsistence of entities. Trigg rejects Rescher's conceptual idealism, which maintains that since without mind there would be no way of distinguishing, and mind only sees a chair from a perspective, we cannot say what a chair is "in itself." This view keeps us on one side of the correspondence relation, Trigg says. He proposes that our concepts are "a window on reality." Peirce's "Final Agreement," what reality corresponds to, is (...) an attempt to bring together realism and idealism, but "final agreement" is an ideal limit to enquiry, not a future event. I give the label “ethical pluralism” to the meta-ethical view that competing moral views are valid. I assume that validity is conferred on a moral view by its satisfying the relevant meta-ethical criteria in a maximally satisfactory way. If the relevant meta-ethical criteria are based on something roughly like the wide reflective equilibrium model, then ethical pluralism is likely to be correct. Traditional moral views do not grant exemptions from their own binding rules or principles to agents – should any (...) exist – who adhere to a competing valid moral view. Given the usual conception of accepting a moral view, an ethical pluralist cannot honestly accept a traditional moral view. Consequently, I argue, an ethical pluralist is committed to the view that all traditional moral views are invalid. Given the likelihood of ethical pluralism, this conclusion is alarming. I set forth a weak conception of accepting a moral view that is designed to allow an ethical pluralist honestly to accept a traditional moral view. In particular, my conception is designed to explain how someone can (a) be guided by the view that she accepts; (b) accept her own moral view while rationally not accepting competing views that she thinks are equally valid; and (c) not be prepared to prescribe morally to those who are following other valid views. Central to my formulation are what I call a stance of modest respectful disapproval toward other people’s wrong behavior, together with acceptance of decisive moral reasons for oneself that are generated by the valid moral view that one accepts. Climo and Howells argue that a comparison of counterfactual statements is the best approach to causation in historical analysis. In historical explanation, it is often difficult to distinguish causes from effects, real causes from potential ones, and epiphenomena from either causes or effects. The symbolic statement "A causes B" describes the actual world. Two statements using the parameters A and B may be formed which do not describe the actual world. By determining which of the statements, "If not-A then B" (...) and "If not-A then not-B," is closer to the actual world, one can conclude whether A is a cause of B. Despite their claims, however, Climo and Howells do not prove that their method is superior to others in dealing with effects and preemption. Their method also has internal difficulties when dealing with epiphenomena and relations to the actual world. (shrink)

The paper is an attempt to make sense of Hegel's notion of aufheben. The double meaning of aufheben and its alleged ?rise above the mere ?either?or?; of understanding? have been taken, by some, to constitute a criticism of the logic of either?or. It is argued, on the contrary, that Hegel's notion of aufheben, explicated in its primary and philosophical context, turns out to be a substantiation of that logic. The intelligibility of the formula of either?or depends, for example, on the (...) categories of Being and Not?Being. But if these categories are regarded as particular finite determinations themselves subject to the formula of either?or, then the formula, far from being intelligible, ?falls apart?. Hegel is arguing, in other words, that if we are to substantiate the logic of either?or, we must, at the same time, ?rise above? that logic. The role of aufheben is then considered in the special sciences. Here it is argued that we must distinguish between empirical transitions, governed by the finite determinations of things, and logical or dialectical transitions, governed by considerations of the intelligibility of the notions involved. Applying the notion of aufheben to the former transitions suggests wrongly that empirical transitions have an objective or philosophic necessity. Finally, the place of ?immanent transformation? in the context of aufheben is examined. It is concluded that if there is to be a transformation, then a distinction must be drawn between thought and its content, but then the transformation cannot be regarded as immanent. (shrink)
